Detailed Description

One entry per local publisher/subscription/service/client, recording which node created it. For services/clients this is still the ONLY source the _by_node graph queries have (AstuteDDS's RPC primitives are not wired into graph_ctx.graph_cache), so those two queries remain local-process-only. For publishers/subscriptions this doubles as the local half of rmw_get_publishers/subscriptions_info_by_topic() (gid + qos), since AstuteDDS's discovery enumeration only reports remote endpoints; node ownership for the _by_node topic queries instead comes from graph_ctx, which also covers remote nodes. Protected by endpoint_mtx.
